Hey Whiskey, I'm glad you came here and asked about bilateral posterior subs-capsular cataracts.
I'm far from an expert but you should know that Posterior subcapsular cataracts are cloudy at back of the lens adjacent to the capsule (or bag) in which the lens sits. Because light becomes more focused toward the back of the lens, they can cause disproportionate symptoms for their size.
Tell your friend not to worry though because cataract removal can be performed at any stage and no longer requires ripening of the lens. Surgery is usually 'outpatient' and performed using local anesthesia. About 9 of 10 patients can achieve a corrected vision of 20/40 or better after surgery.
Hope that helps.

I knew my friend would find an answer here
He has a question. Scalpels or laser (type stuff) to cut my, err his eyeballs?

Having some dude freehand it with a scalpel is ok if that's all you can afford.
I would definitely go with a robot laser mapped from a optical coherence tomography...but that's just me.

Yes, he didn't think scalpels were the current tool of choice.
Another quickie: Do they use sutures or something like a
glue these days?

The whole clockwork orange style eyes pinned open whilst shot with a laser put me off somewhat.
How's your night vision? Don't some people end up with poor night vision afterwards?

Night vision is great. Best thing I've ever done. Complete change in quality of life. Of course, I was -7.50 in both eyes, so the change was drastic...
