sm0k3r's "Pitch Blue" is okay.. Practically all of Lukin's maps get good play, although "monsoon" is clearly his best work... There's SCADS of great custom maps out there, if you look.
At LANs I've attended recently, some maps that did well are:
RolaDM
Monsoon
DM Prison Abuse
Heat
High and Dry
That's all I remember, at least..
I think all of them are searchable on quake4.filefront.com.. I'm not finding all the links, but "heat" is mine, so you can get it here:

Can I just use si_mapcycle or do custom maps require being loaded in the actual custom map config file?

si_mapcycle, g_mapcycle, rcon or callvote - all methods work

Any chance Q4Max will come out with a map rotation system? Something with random map cycle options?
