Grapple is only for CTF. It doesn't belong in any other gametype.
Besides, no Quake game has ever had a grappling hook out of the box, let alone in DM, TDM or 1v1.

Grapple is used in FT quite frequently too, but that's a mod too, obviously. I like railjumping more than grappling anyway.

yeah but in FT as well, IMO, it only detracts from the core gameplay. It really doesn't add all that much to the game.
In CTF, where it's really about chasing other people, the grapple really adds an unique oppertunity to gain extra speed at a risk. Chasing the flag carrier or fleeing with the flag is an important aspect of the gametype. There's no such thing in plain (T)DM type of gametypes, including Freeze Tag.
Additionally, FT uses an offhand grapple, which means you don't have to switch to it like it's a weapon. This removes the risk that the use of an onhand grapple has. It just doesn't involve any trade off for the added mobility which makes offhand grapples rather dumb.

Quake 3 had grapple out of the box, you just had to enable it.

And Q2CTF had a grapple too.

Lithium DM had a grapple. No reason that the grapple has to be absent from DM style games.

grapple is fine, but only if it's an onhand weapon. There needs to be a tradeoff for the added speed and movement.

There doesn't "need" to be. Ideally the item should be configurable server-side in any given mod, for main-hand or "off-hand" use. That way everyone can gravitate to the servers that use their own preferred style.
Loki's Minions CTF was great fun with its off-hand grapple. Threewave CTF was great fun with it's main-hand grapple.
I've seen some incredible stuff achieved by a person using both a main-hand grapple and off-hand grapple at the same time. Insane stuff. =)
