what version of mouseware are you using? i have the latest and no problem binding anything to the mousewheel.
also, to utilize the 4, 5, 6, etc buttons, change their function in windows to a keystroke, like F10-12 or something.

Happens sometimes with valid keys. If, after deleting and re-entering your key, it still doesn't work: pull down the console from the menu and type /timescale 100 and connect to a server. hey this guy might actually know what hes talking about. :shrug: yeah that happened to me for the longest time ...
